New York Medicaid for people with disabilities
New York Medicaid covers low-income residents including people with disabilities. If you are over 65, blind or disabled, or need long-term care, you generally apply through your Local Department of Social Services (in NYC, HRA); others can use NY State of Health or myBenefits. Applicants without an SSA disability finding can be certified through the State Disability Review Unit. OPWDD & home-and-community waivers
The Office for People With Developmental Disabilities (OPWDD) serves people with intellectual/developmental disabilities through its Front Door and Comprehensive HCBS Waiver (day and community habilitation, respite, supported employment, and more). Call the Front Door at 1-866-946-9733. OPWDD Front Door. Two other Department of Health waivers — NHTD (Nursing Home Transition & Diversion) and TBI (Traumatic Brain Injury) — help people live in the community; apply through your Regional Resource Development Center. Home care: CDPAP & Managed Long Term Care
CDPAP lets eligible Medicaid members hire and direct their own personal assistant — often a family member or friend. CDPAP. Adults who need community-based long-term care for 120+ days generally enroll in a Managed Long Term Care plan through New York Medicaid Choice (1-888-401-6582). MLTC.
Jobs, home mods, rides, tax, parking
- ACCES-VR provides vocational rehabilitation; SSI/SSDI recipients are presumed eligible. Apply.
- Access to Home grants pay for accessibility modifications (ramps, roll-in showers) through local program administrators. HCR Access to Home.
- Medicaid transportation to medical appointments — book 72 hours ahead (downstate 844-666-6270; upstate 866-932-7740). Details. In NYC, Access-A-Ride is the ADA paratransit; elsewhere use your regional transit authority.
- Property-tax exemption for persons with disabilities and limited income — up to a 50% assessed-value reduction, set locally. File Form RP-459-c with your local assessor. NYS Tax.
- Disabled parking permit — complete Form MV-664.1 and submit to your local city/town/village clerk (free). NYS DMV.
Local disability help, advocacy and independent living
Your state and federal agencies decide whether you are eligible for a benefit. These organizations do something different: they help you understand your rights, apply, appeal, and live independently — and most cost nothing to use. Some are government agencies and some are independent nonprofits designated under federal law; each card says which. Share personal information only with an official agency or an organization it designates.
